The correct procedure would be to do them all, on both heads. This would include, doing valves, seats , guides, and seals. Check the heads for warpage, if out of spec resurface both. Basically, rebuild both heads. Of course you should measure or have a shop measure to see if what is good and what is out of spec. You could probably get away with just doing the guides and seals (on both heads) but I wouldn't do it that way. When it comes to tearing your engine down that far, you really only want to do it once- so do it right. As for what caused it, I would check that valve over really good. I see you said it slid out really easy, that's good. I would also check the valve seat and the guide clearance, if there is too much play I could see the valve rocking back and forth in the guide damaging the outer edge as the picture shows. I think the truth will be in your measurements.
